7TH GRADE

INTERSCHOLASTIC SPORTS

Any athlete that wishes to participate in a 7th grade interscholastic sport need to have on file the following:

1. Current Medical Physical

2. Activity Permission Form/Insurance Coverage

3. Parental Consent Form

3. ASB Card : $15.00 (collected during school picture day)

4. Pay to Play $10.00 per sport.
